01 Month 01 — Weeks 1–4
Legal & Credentialing

The entire legal and credentialing backbone of your practice is filed and in motion. This is the foundation everything else sits on.

02 Month 02 — Weeks 5–8
Digital & Payments

Your practice goes live online. Billing infrastructure turns on. The digital front door opens while credentialing processes in the background.

03 Month 03 — Weeks 9–12
Acquisition & Launch

The pipeline opens. Patients arrive. You transition out of employment with revenue on the calendar — not after.

All of this happens while you're still employed.

Months 1 and 2 run entirely in parallel with your current job. Entity registration, credentialing, website build, and billing setup don't require you to resign. Most practitioners don't give notice until patients are on the calendar in Month 3 — with a paycheck still arriving.

Month 01 · Weeks 1–4

Legal & Credentialing

The backbone of your practice is filed, submitted, and in motion. Every step in Month 2 depends on what gets done here — which is why nothing else starts until this is complete.

  • Business Entity Formation

LLC or PLLC registered in your state. Articles of Organization filed. EIN issued by the IRS the same day. Business banking account opened.

  • NPI Type 1 & Type 2

Your individual NPI verified and updated. Group NPI (Type 2) applied under your new business entity — required before any insurance billing can occur.

  • CAQH ProView Profile

Your CAQH profile built or updated correctly — the single data source that most payers pull from during credentialing. Getting this right in Month 1 prevents delays in Month 3.

  • Payer Credentialing Applications Submitted

Applications submitted to your target payers. Processing takes 90–120 days — starting in Month 1 means you're credentialed by the time you see your first patients.

  • Malpractice Coverage & DEA

Independent practice malpractice policy confirmed. DEA registration verified or transferred to your new entity where required by specialty.

  • Employment Contract Reviewed

Your current contract reviewed for non-competes, restrictive covenants, patient solicitation clauses, and notice requirements — before you make any moves.

Month 02 · Weeks 5–8

Digital & Payments

While credentialing processes in the background, your entire digital presence and payment infrastructure goes live. By the end of Month 2, patients can find you, book you, and pay you.

  • Professional Website Launched

A conversion-optimised clinical website — built on your domain, HIPAA-compliant, with your specialty, services, bio, and booking widget live.

  • Stripe & Insurance Billing Configured

Stripe set up for HIPAA-compliant cash-pay collection. Insurance billing account activated with your clearinghouse. You're ready to get paid from Day 1.

  • Scheduling & Telehealth Platform Live

HIPAA-compliant scheduling platform configured and integrated with your website. Telehealth room live and tested. Patients can book directly.

  • EHR Configured

Electronic health records platform selected and configured for your specialty. Note templates, workflows, and billing codes ready before your first appointment.

  • Intake, Consent & Practice Documents

Patient intake forms, informed consent documents, practice policies, and HIPAA notices — built, reviewed, and embedded in your scheduling workflow.

  • Directory Profiles Published

Psychology Today, Zocdoc, Headway, Alma, and specialty directories — profiles built and published so patients can find you before the first ad campaign runs.

Month 03 · Weeks 9–12

Acquisition & Launch

The pipeline opens. Campaigns go live. Referral relationships are activated. First inquiries arrive. You manage your transition out of employment with patients already confirmed on your calendar.

  • Meta & Google Ad Campaigns Launched

Targeted ad campaigns live on Meta (Facebook/Instagram) and Google — built for your specialty, your location or telehealth service area, and your target patient population.

  • Referral Network Opened

Your referral map built. Target referrers contacted — PCPs, ERs, schools, community mental health organisations, and specialty clinics depending on your practice model.

  • First Inquiries Managed

Your intake workflow tested end-to-end with real inquiries. Phone scripts, scheduling flow, insurance verification, and intake packet delivery — all running before you give notice.

  • First Patients Seen

Revenue on the calendar before your employment ends. The goal is a confirmed caseload in place — not a practice waiting for patients — when you hand in your notice.

  • Employer Transition Executed

Notice given on the right timeline. Transition managed in compliance with your employment contract. Patient communication handled correctly to protect your relationships and your licence.

  • 30-Day Post-Launch Review

Four weeks after launch: caseload assessed, campaign performance reviewed, billing reconciled, first insurance claims tracked, and Month 4 growth plan set.

Month 03 Progress Launching
  • Meta campaign live
  • Google campaign live
  • Directory listings boosted
  • First inquiries — 3 active
  • Referral network open
  • First patient appointments confirmed
  • Notice given to employer
  • First patients seen
  • 30-day post-launch review
The Infrastructure

Three pillars. All of them built.

The 90-day framework isn't a course about building a practice — it's the buildout itself. Each pillar is a functional piece of operational infrastructure, not a plan or a checklist.

Common Questions

How the framework actually works in practice.

Yes — and this is one of the most important sequencing decisions in the entire framework. Payer credentialing takes 90–120 days from submission. If you wait until your website is live to start the process, you'll have a fully built practice with no ability to bill insurance for another three months. Starting in Month 1 means the processing window runs in parallel with your build — and you're credentialed by the time your first patients arrive.
Yes. The entire framework is designed to run alongside your current job. Month 1 and Month 2 work — entity registration, credentialing, website build, billing setup — doesn't require you to be available during business hours. Most of it is paperwork, platforms, and applications. You give notice in Month 3 when patients are on your calendar and the practice is generating revenue.
The framework adjusts to your state's current rules. In full practice authority states, you file and operate independently. In restricted states, we build the collaborative agreement into the Month 1 sequence — it becomes one more item that gets handled, not a blocker that stops the launch. You should confirm your state's current requirements with a licensed healthcare attorney before finalising your structure.
LLC filing: 1–3 business days in most states. EIN: same day online. NPI Type 2: 3–5 business days. CAQH: 1–2 weeks to complete and verify. Payer credentialing: 90–120 days (processing time, not your time). Website build: 2–3 weeks. Stripe setup: 1–2 days. Ad campaigns: 3–5 days to build and launch. The 90-day timeline is built around these actual processing times — the work fits because the sequence is designed to fit.
First inquiries typically arrive within 7–14 days of ad campaigns going live. The goal is confirmed appointments before you leave employment — not patients at the exact moment employment ends. The 30-day post-launch review is specifically designed to address acquisition performance and adjust campaigns if the first month underperforms target.
It depends on your tier. In Blueprint, you execute everything using the course curriculum and templates. In Accelerator, you execute with live coaching, direct team messaging, and expert calls at each stage. In Launch, our team handles the entity filing, website build, campaign creation, and credentialing submissions — you review and approve milestones. The clinical work — the patients, the appointments, the prescribing — is always yours.
